Pathway 01

Learning Disabilities Support

Reliant Care provides specialist residential support for adults with mild, moderate and severe Learning Disabilities including those with associated complex health needs, communication differences and co-occurring physical conditions.

Mental Health Specialist Residential Care

We provide specialist residential support for adults with complex and enduring mental health conditions including psychosis, personality disorders, dual diagnosis, and treatment-resistant presentations delivered through a recovery-focused, trauma-informed model that restores hope and rebuilds life skills.

Pathway 04

Forensic Step-Down & Community Transition

For individuals stepping down from medium or low-secure inpatient settings, or those subject to Conditional Discharge or Community Treatment Orders, we provide robust, MDT-linked community placements that reduce risk, rebuild lives and support safe, sustained discharge in line with the NHS Transforming Care programme.

  • Experienced in supporting individuals subject to MHA s.37, s.41, CTO and conditional discharge
  • Specialist forensic risk assessment, safety planning and proactive risk management
  • Close liaison with Community Forensic Teams, Responsible Clinicians and probation
  • Offence-specific intervention support (arson, aggression, sexual offending pathways)
  • Transparent reporting; tribunal-ready support documentation and evidence packs

How it works

Our referral & placement process

We understand that referrals into specialist care are time-sensitive and complex. Here is exactly what happens when you contact us — clearly, step by step.

Initial contact & suitability conversation
Contact our Specialist Services Referrals team by phone or email. We will have a confidential, no-obligation conversation to understand the person's needs, current placement situation, legal status and commissioning authority. We respond to all referral enquiries within one working day.
We request and review all relevant documentation including health and social care assessments, risk formulations, psychiatric reports, current care and support plans, and (where applicable) legal orders and tribunal documentation. Our clinical lead reviews within 48 hours.
Where we identify a potential match, we arrange a pre-placement visit and assessment — involving the person (where possible), their family, current care team and the proposed keyworker. We assess compatibility with existing residents, staffing expertise, and the home environment's suitability to meet the individual's specific needs.
Before any placement begins, we co-produce a detailed support plan, positive behaviour support framework, crisis plan, and transition schedule — working alongside the person, their family and the full MDT. Transition is managed carefully, at the person's pace, with gradual familiarisation visits where appropriate.
Once the placement begins, our service manager provides an intensive settling-in period. We hold formal placement reviews at 6 weeks, 3 months and 6 months — and quarterly thereafter — with the person, their family, social worker and community clinical team. We proactively flag any emerging concerns to commissioners within agreed timescales.
