What is Floating Support?

Supporting you to live independently, in your own home.

Reliant Care's Independent Living & Floating Support is available to adults living in the community who need help maintaining their accommodation and their ability to live independently. Depending on your needs, we can support you from 6 months to 2 years.

Our services are specifically designed to enable individuals with severe and enduring mental health conditions and learning disabilities to live independently. Whether in their own house, flat, or shared living environment. We focus on promoting maximum independence to meet your specific needs.

Health, Wellbeing & Daily Living

We support you to maintain your health and look after yourself day to day. Our staff help with the practical and emotional side of life at home from preparing nutritious meals to building healthy routines that give your days structure, purpose and confidence.

Tenancy Support & Financial Skills

Keeping a home requires a range of practical skills that can feel overwhelming. Our staff work alongside you to build the confidence and ability to manage your tenancy, handle money wisely and take control of your finances so you can maintain the stability you've worked hard for.

You are referred to us via a local authority or healthcare professional. We conduct an initial needs assessment to understand your situation, goals and what matters most to you.

Creating Your Plan

Together, we co-produce a unique, person-centred support plan — outlining the type of support, how much, how often, and how it fits around your daily life.

Active Support

Your dedicated support worker starts working with you, providing flexible support during the day, evenings or weekends — at the times that suit you best.

Review & Progress

We regularly review your plan with you to track progress, adapt support as your needs change, and celebrate the independence you are building every day.
