A live system that puts the right support in the right place at the right time

The DSR is a live, regularly updated NHS database identifying children, young people, and adults with a learning disability, autism, or both who are at risk of hospital admission and coordinating proactive community support around them before crisis strikes.

Who the DSR Is For

A focused tool for those with the highest need

The DSR carries a strict and important boundary. It is exclusively for individuals with a formal diagnosis of a Learning Disability (Intellectual Disability), Autism, or both who are also experiencing escalating needs that put their community placement at risk. Reliant Care's team supports individuals across all age groups to access this pathway.

  • Formal Learning Disability (Intellectual Disability) diagnosis

For young people up to the age of 25 at Amber or Red on the DSR, Reliant Care supports the allocation of a dedicated NHS Keyworker — a single named professional who handles all coordination between schools, social workers, and doctors. Families stop having to repeat their story. And when escalation requires an independent Care and Treatment Review, we are there throughout.

How Reliant Care monitors and responds to risk

Every individual on the DSR is assigned a standardised colour-coded status. Reliant Care's team reviews each case at multi-agency panel meetings every two to four weeks with immediate protocols activated for those at highest risk.

Red · High Risk

Immediate Intervention

Hospital admission is imminent without urgent community action. Reliant Care activates crisis protocols and fast-tracks an independent C(E)TR. Intensive support is deployed within 48 hours.

Proactive Adjustment

Significantly increased risk of deterioration or placement breakdown. Our team proactively adjusts care packages, triggers keyworker allocation for under-25s, and intensifies community support.

The individual has been admitted to a specialist hospital. Reliant Care immediately begins building a funded community discharge package with a mandatory review mandated within six weeks of admission.

Getting started

How to access Reliant Care's DSR support

Whether you are a commissioning social worker, NHS discharge coordinator, school SENCO, family carer, or self-referrer. Our DSR team responds to every enquiry within one working day and guides you through every step of the process.

Referrals can be made by a healthcare professional, social worker, or school SENCO. Local ICBs also allow self-referrals. Reliant Care can support you in identifying the right pathway and making the first contact.

Our team conducts a detailed initial assessment to understand the individual's diagnosis, current needs, level of risk, and existing care arrangements. This helps us determine the appropriate DSR risk level and which agencies need to be coordinated around the individual immediately.
Informed consent is obtained from the individual (or parental consent for children). If an adult lacks capacity, a formal Best Interests decision is made under the Mental Capacity Act 2005. Choosing not to join the DSR will never affect the standard care someone receives. Once consented, the individual is registered and assigned a risk colour.
The individual's case is brought to the next Multi-Disciplinary Team panel usually within two weeks. Reliant Care advocates directly for the individual, ensuring their voice, choices, and communication needs are at the centre. Immediate actions are agreed across NHS, social care, and education with clear owners and timescales.
We review risk status at every panel and continuously monitor wellbeing, physical health, meaningful daily activities, and medication use. If an individual's situation improves, their risk level decreases. If things escalate, Reliant Care activates the appropriate protocol immediately C(E)TR, intensive support, or crisis team — before hospital becomes the only option.
