The mental health service at Reliant Care provides accommodation to individuals with common to complex mental health needs, including co‑morbid and transdiagnostic disorders. We follow a patient‑centred approach and offer psychotherapy programmes such as cognitive behavioural therapy (CBT).

The Learning Disability service at Reliant Care provides accommodation to individuals with moderate to complex learning disabilities. We offer a range of activities tailored to each person’s specific needs, including positive behaviour support and life‑skills development.

The Psychology Team at Reliant Care offers 1‑1 and group sessions using a protocol‑driven approach. Our psychotherapy interventions include cognitive restructuring, behavioural experiments, emotional regulation, distress tolerance, mindfulness, and SMART goal planning. We also provide music, dance, and art classes to enhance creativity and support healthier coping strategies.

The MOJ service at Reliant Care provides accommodation to both males and females with forensic or criminal histories. Our intensive programme includes psychological talking therapies, supported employment, and life‑skills development.

Dynamic Support Register (DSR) and Care (Education) and Treatment Reviews (C(E)TRs) to prevent avoidable hospital admissions and promote least restrictive care. The DSR utilizes a RAG-rating system to stratify admission risk, labeling individuals as Red (immediate risk), Amber (increasing risk), or Green (stable) to trigger timely multi-agency interventions. When an individual is rated Red or escalating Amber, a person-centered C(E)TR is considered to coordinate urgent admission avoidance or robust discharge planning. The model features a Step-Up pathway to stabilize active crises in the community, alongside a Step-Down pathway designed to build independence and reduce restrictive practices following an inpatient stay. Ultimately, all transition decisions require collaborative Multi-Disciplinary Team (MDT) coordination, with formal referrals mandating a clear DSR status, current risk ratings, and comprehensive supporting documentation.

Our Step-Up and Step-Down services provide a flexible pathway designed to prevent avoidable hospital stays and support smooth transitions back to independent living. Step-Up support offers immediate, short-term care to stabilize individuals experiencing a crisis or sudden health decline, avoiding unnecessary hospital admissions. Step-Down support assists individuals recovering from an inpatient hospital stay, focusing on rehabilitation, regaining daily living skills, and safely rebuilding their independence. Managed by a dedicated Multi-Disciplinary Team (MDT), every care plan is fully personalized, person-centered, and safely coordinated. This structured approach ensures continuity of care, reduces restrictive practices, and empowers residents at every stage of their recovery journey.

Mead House Day Academy, developed by Reliant Care Limited, is an innovative, outcome-driven service designed for learning disabled and autistic adults, as well as individuals with complex behaviors or mental health needs. Moving away from traditional, passive day care, the Academy operates from 8:00 AM to 8:00 PM, seven days a week, all year round, transforming daytime support into meaningful, skill-based, and therapeutic progression opportunities. Its core mission focuses on stabilizing community placements, reducing restrictive practices, and proactively managing risks to prevent unnecessary crisis-driven hospital admissions. Operating in strict alignment with national NHS England guidelines including Dynamic Support Registers (DSRs) and Care (Education) and Treatment Reviews (C(E)TRs) the service provides transparent, measurable outcome data to commissioning partners through a robust quality governance framework. To achieve these outcomes, the Academy features an array of specialized, modern environments, including therapeutic art and music rooms, sensory and calm spaces, a tailored gym and dance studio, and a physical health check room designed to lower medical anxiety. It also boasts real-life training grounds such as industrial kitchens feeding an on-site café, an indoor-outdoor garden managed with the local horticultural society, and a high-tech "Smart House" living lab equipped with assistive technology for functional independent living assessments. Service delivery follows a structured two-phase path: Phase 1 establishes a comprehensive baseline through personalized travel, living, and vocational profiling to build a Life Skill Attainment Plan. Phase 2 executes these goals through practical, certified skill-building modules—such as financial budgeting, office-based IT competencies, and accredited Level 2 Food Hygiene courses ultimately preparing participants for meaningful volunteering, independence, and supported employment.
