Reliant Care provides support that is based around each resident’s individual needs. Our residents have varying needs but all enjoying living within a secure, homely and caring environment.

Many of our residents are on an enhanced level of Care Programme Approach (CPA) and will have regular visits from care co-ordinators, attend local outpatient appointments and liaise regularly with the mental health services to ensure they are receiving the best quality on-going support.

Support

Reliant Care provides support that is based around each resident’s individual needs. We acknowledge that the same diagnosis may have a different impact on each individual client and adapt their support plan accordingly. We are responsive to the cultural, gender and any specific needs of the resident in developing their support plan. Whatever the need, support is provided with patience and understanding, whilst at the same time, provided within appropriate boundaries.

Each resident’s support plan is exclusive, with the aim being to promote good health, increase independence and promote good quality of life. We actively encourage residents and appropriate significant family members, and support network to participate in the support planning process. Each resident is registered with the local primary health care service. If they wish to remain with their current GP and the GP is willing, residents may choose this option.

Resident’s health needs, particularly medication, are kept under close and regular review. In recognising the importance of personal, individual and resident focused care, every effort is taken to ensure good communication between all members of the care team, be they nurses, carers, relatives or doctors.

An Ideal Setting

Our intensive Support Unit at Beaconsfield Road is a spacious two storey home situated in the residential area of Southall, Middlesex with vacancies for 14 adults. It is within walking distance of local amenities, with easy access to comprehensive shopping, educational and recreational facilities, offering good public transport including a mainline railway station.

Beaconsfield Road is also the location of our head office.

In meeting the daily challenges that residents face, we strive to provide support from staff and individual programmes which are geared to meet every resident’s needs.

Development

It is our aim to assist each resident in maintaining the skills that they come to us with, and the development of skills that will enable their full participation in the life of the immediate and wider community. We work with residents to develop competency in areas that will enhance the ability for self-determination and acceptance. The changing needs of our residents are assessed continuously, and programmes are adjusted accordingly, to fully support, enable and empower our residents to achieve their full potential.

Social Skills

Socialisation and new life experiences are of paramount importance to an individual’s personal development. Skills addressed include, basic social, shopping and personal money management skills, environmental awareness and road safety.

Day Services

All day activities are formulated on an individual basis according to assessed need and many are community based. The service promotes individual choice, privacy, dignity, rights and independence.

Leisure

We encourage the development of a wide range of leisure interests. A full range of leisure activities and facilities are accessed, and residents are provided with an enjoyable exercise programme.

Social Inclusion

Reliant Care is keen to eliminate the risk of boredom, so we have a very engaging social inclusion programme to encourage residents to have a full and meaningful day. We feel that a regular change of atmosphere helps them to deviate away from their usual routine especially if this involves alcohol and substance misuse. The main purpose of our social inclusion programme is to empower service users to re-integrate into the community and improve their quality of life.
